Requirements for submitting a complaint
A consumer may lodge a complaint against a credit provider by submitting a completed Form 29 to the NCR by fax, mail, e-mail or by telephone;
Complaints lodged by telephone and e-mail may be lodged only by the complainant, not someone on behalf of the complainant;
If a person completes and submits Form 29 on behalf of the complainant, the complaint must be accompanied by a written consent signed by the complainant;
A third party may act on behalf of a complainant only if the complainant has given written consent;
A written consent provided by the complainant must contain the name of the third party; name of the credit provider; date of signing of the agreement to which the complaint relates; and details of the complaint.
